Summer Internship
Compulsory internships can only be undertaken at the end of the 3rd year.
If the internship will be carried out in a private-sector organisation (i.e. the host is not a public institution), the following form must be completed:
Unemployment Fund Contribution to Internship Wages Form (all departments)
(Microsoft Office must be installed on your computer to view and edit these forms.)

Students who will do an internship abroad (with or without Erasmus) do not need to take any action here. Erasmus interns must apply through the Erasmus Office; non-Erasmus interns abroad have no further obligations under this procedure.
Guidelines for Completing the Forms

Internship Application Form

Download, fill in your personal details, host-company information, and, if known, the start and end dates.

Print two copies.

Submit the completed form to the Department Secretary for signatures from your Department Internship Coordinator and the Head of Department.

Download the Unemployment Fund Contribution to Internship Wages Information Form and fill it in completely on your computer.

After obtaining the departmental signatures, take both signed copies of the Internship Application Form to the host company for approval.

Have the host also sign/stamp the Unemployment Fund form.

All relevant sections must be fully completed, stamped, and signed by an authorised company representative.

After company approval:

Leave one original copy of the Internship Application Form with the company.

Submit the other signed copy together with the signed Unemployment Fund form to the Department Secretary.

Photocopies or faxes will not be accepted.

If you are interning outside the city, send the originals by courier or post after company approval (coordinate with your Department Secretary).

If the company has already issued you an official letter of acceptance (bearing stamp and signature), you may attach the original of that letter to the Application Form and submit it without the company section being filled in.

Copy of both sides of your ID card

One passport-size photograph
NOTE:
All internship documents must reach the Department Secretary at least 15 days before the internship start date. The date of receipt, not the postal dispatch date, is binding. The department is not responsible for postal delays.
After the Internship

You must submit an internship logbook completed in the format below to your academic supervisor.

All students must prepare a laboratory report following the internship-logbook procedure.

The Intern Evaluation Form on the last page must be filled in, stamped, and placed in a sealed, signed envelope by the supervisor, then submitted together with the logbook.
